About FormWorks

FormWorks® allows schools to create custom online forms for tracking data digitally.


  • Forms can be stand-alone, generating reports on collected data.

  • Forms can also integrate with other DMAC applications like Appraisals, Student Portfolio, Learning Plans, and PlanWorks.

  • Approved Authors must create form templates for them to appear for others to complete.

Managing Authors

District and Campus administrators can manage settings and select FormWorks Authors.

  • Go to Settings in the top navigation bar, then click Authors.

  • Select a Location from the drop-down on the left.

  • Click the Add New icon to add authors who can create templates.

  • To remove an Author, click the Delete icon.


Managing Templates

  • A blue box with a number next to an Author’s name shows the total templates assigned to that user.

  • Click the box to view form titles or reassign templates to another user.

  • Select one or more templates, then click Reassign Selected. Choose the Campus and Name, then click Reassign.

  • Uncheck the Active checkbox to view forms without assigned authors.
