FormWorks offers two options for creating templates:
Start a new template
Copy an existing form
Starting a New Template
To create a new template:
Select Templates from the top navigation bar
Click the Add New icon
Choose Start a blank template
Setting Template Details
Enter the template name
Select the Type from the drop-down
Necessary if attaching to a DMAC application like Student Portfolio, PlanWorks, Learning Plans, or Appraisals
For a stand-alone form to collect data, select FormWorks
Use the Location drop-down to set accessibility for the entire district or selected campuses
(Optional) Select a Category
District or campus admins can create custom categories under Settings
Adding Approvers
If the form requires a review:
Click Add Approver
Use the drop-down to find district or campus logins, or search by first or last name
Drag names into the selected approvers box
Multiple approvers can be added, and their order rearranged
Click Save
To remove an approver, click the X icon
To change the order or add names, click Edit
Granting Edit Access to Other Authors
Click the Add Authors link
Use the location drop-down to select names
Click Select
Once all options are set, click Save
Designing the Form
FormWorks offers endless possibilities for form design:
Use the boxes from the left menu to drag and drop field types
Organize fields by dragging them into new locations
Hover over each added field and use the Settings icon to add content.
Finalizing the Template
Once the form is designed, use the icons in the top right:
Preview: Shows how the form functions for users
Publish: Makes the template live for form completion and data collection
Once published, the template is read-only
To edit, click Unpublish (available until a form is completed)
Print: Saves or prints a PDF version of the template
