Searching Templates in FormWorks
Approved Authors must create and publish templates to collect data in FormWorks.
Access Templates:
Click Templates in the top navigation bar.
Filter Templates:
Filter by Type/Application, Location (district or campus), Status (active, published, deleted, archived), Approval Path, completed forms, or Category.
Use My Templates Only to view templates tied to your DMAC login.
The total number of results is shown in parentheses next to Results.
Use the search box to find templates by full or partial title, then click Find.
Template Information:
Templates are listed with the assigned category (in green), title, location, creation date, and Author.
A blue box indicates if a form requires approvals.
Template Actions:
Publish Toggle: Green indicates the template is live; gray means it's unpublished. Toggle it on or off until a form is completed.
Forms Column: If the number is greater than zero, the template cannot be edited or unpublished. Copy or archive it to make changes.
Settings (gear icon): Update name, type, location, category, Author, assign/edit Approver(s), and add Authors with edit access.
Copy: Duplicate and rename templates with completed forms to make changes.
Archive: Deactivates the form, preventing further data collection. Filter by archived forms on the left.
Edit: Modify the template until it’s published. Unpublish to edit if no form has been completed.
View (magnifying glass): Open a read-only version of the form.
Delete (red x): Remove a template if no forms are completed. This option disappears once forms are tied to the template.
